Objective:Toassessthepossibilityofone-stagereconstructionofseveresofttissuedefect nlowerextremities.Method:Thediagnosisandtreatmentfor43patientswithseveresofttis suedefectinlowerextremitiesfrom December1998to December2008 wasretrospectivelyre viewed.Results:Reconstruction wassuccessfulin42 patientsandfailedin onepatient whose woundhealedafterthestumpwasshaped.Ofthe42patients,38recoveredfrom one-stagere construction.Infourpatients,thedefectswerecoveredthoroughlybysurvivingflapsbutsmall necroticwoundsthatoccurredinperipheraltissueduetoseveredisruption wererepairedbyskin transplantationandlocalsmallrotationflapsinsecond- stage.After1– 36 monthsfollowup, restorationofshapeandfunctionoflowerextremities wassatisfactory.Conclusion:Severe softtissuedefectsinlowerextremitiesareusuallycausedbyhighenergetic,complexand multi pleinjuries.Witheasyandsimpleinternalorexternalfixation,andskilful microsurgicaltech niques,especiallyutilizingtheflaptechnique,a woundcanlikely berepaired with one-stage procedure.
